My client wants a "Contact Us" widget similar to the one in the attached picture.

How am I able to make something like this? Does PageLines have a Drag&Drop feature that is like this, a recommended plugin, or widget where I can put this on my clients site?

Hi,
When it comes to contact form, I recommend the Gravity Forms plugin, although it is a premium plugin it is the best out there in my opinion. The plugin comes with a widget that allows you to select a form that you have created and add it to your sidebar.
